you might need a new transmission...
It sounds as though the engine isn't firing properly, it could be anything from a blocked fuel filter, blocked injector, any number of things! Take it to a garage, or get it towed if it won't make it that far!
Vacuum leak, which is relatively minor. There is an obvious obligation on your part to check the motor oil level as well, so it doesn't seize or do costly damage.
